Orthodontic Injuries – What to Do
Braces, retainers, and other orthodontic devices are an important part of achieving a healthy smile. However, accidents, falls, or collisions can sometimes cause injuries to the mouth and surrounding tissues. If that happens, Paramount Dental is here to ease your pain and provide immediate care.
Common Orthodontic Injuries
Orthodontic-related accidents may result in:
- Cuts, punctures, or lacerations from brackets or broken wires
- Trauma to the face, teeth, or mouth
- Swelling or redness in and around the mouth
- Intense or persistent pain
- Damaged brackets, wires, or retainers
- Loosened or knocked-out teeth
- Broken mouthguards
First Aid for Orthodontic Injuries
If you’ve suffered an orthodontic injury, take these steps until you can see a dentist:
- Rinse with warm saltwater: Helps clean the area and reduce irritation.
- Apply a cold compress: A cold washcloth or ice pack reduces pain and swelling.
- Eat soft foods: Avoid hard or crunchy foods that could worsen the injury.
- Manage pain safely: Use over-the-counter medication such as ibuprofen or acetaminophen. Avoid aspirin, as it may increase bleeding.
Then, contact Paramount Dental right away at (916) 898-0003 for urgent care.

Orthodontic Injury FAQs
Is a poking wire an emergency?
A poking wire is typically not an emergency. Cover the wire with orthodontic wax and call us. If pain is severe or you suspect a tooth injury, seek care right away.
Can I trim a wire at home?
This is not recommended; however, if you cannot be seen promptly, and the wire is causing injury, then it may be necessary. Sanitize small nail clippers, trim the very tip of the wire, and cover with wax. Schedule a professional fix as soon as possible.
What if a bracket comes loose?
If the bracket is still attached to the wire, place wax over it to hold it steady and avoid chewing on that side. If it detaches, save it and bring it to your visit.
My aligner cracked or I lost it—what now?
Be sure to save the cracked tray. If it’s comfortable, wear the previous aligner to maintain your bite. Contact us for guidance on the next steps.
How do I relieve mouth sores from braces?
Place orthodontic wax over the rough areas causing irritation. Saltwater rinses and a soft diet will also help. The good news is that most sour spots improve in a few days with these recommendations.
When should I go to the ER?
If you have head or face trauma, uncontrolled bleeding, a tooth that’s very loose or has been knocked out, fever with facial swelling, or trouble breathing/swallowing, seek emergency medical attention immediately.
